When you submit the appointment request form we collect your name, mobile number, optional email address, the department or doctor you choose, your preferred date and time and an optional message. Please do not include medical history or medical records in the form.
How we use it
We use this information only to contact you about your request and to manage appointments. We do not sell your information.
Notifications
Your request is stored securely and our team is notified by email. Email delivery can be delayed, so please call us if you need urgent help.
WhatsApp and phone
If you choose to contact us on WhatsApp or by phone, that communication is subject to those services own terms.
Your choices
You can ask us to correct or delete your request by contacting us using the details on the Contact page.
